Schedule

Accepting New Clients!

To schedule an appointment please call (612) 787-2828 or email.

What’s my first step as a new client?

Before you set up your first appointment, call your health insurance to become informed about what will be covered and what payments you may be responsible for. This only applies if you are planning to use health insurance. Next, you would complete an appointment request and complete the screening form and appropriate assessment form to bring in for your first appointment. Please also bring in your health insurance card and a valid ID. You will receive an email confirming your initial assessment.

Parents or caregivers who schedule first appointments for children age ten and younger are asked to attend the first appointment alone. At the second appointment, the child is asked to join their parents or caregivers.

Appointment Policies

To ensure client care, we ask you to attend your scheduled sessions. Missed appointments or cancellations less than 24 hours prior to the appointment are charged at a rate noted in the Payment Contract for Services.

OFFICE HOURS

Daytime and some evening hours are available,
with variations depending on client schedules.

CLIENT PORTAL

Please fill out the screening form and initial assessment form before your first appointment.
